• Please apply your passport before participation this program. Your valid passport must have at least six (6) months of remaining validity and at least one blank visa page in it.
• Fill out the Site Transfer Form; please note your signature is required on the last page. Download  site transfer form and please submit the Site Transfer Form to Webster Staff once you complete them.
  
• Physical Exam (Immunization).   Download 体检须知
  Immunizations and a student health history are required for all students living in campus housing. The required immunizations and student health history must be complete PRIOR to the student moving into campus housing.  Failure to complete and submit the required immunizations and student health history PRIOR to moving into campus housing will result in a delay in registering for classes and access to campus housing will be denied.   
  Please download and bring this form to International Travel Healthcare Center (Shanghai, Shenzhen and Chengdu) and do Physical Exam.  Student Health and Immunization History Form  for your refrerence. The original  form will  send to you once receive it form St. Louis around end of the March. 

• You will receive an International Certificated of Vaccination orProphylaxis issued by Entry-Exit Inspection and Quarantine THE PEOPLE'S REPUBLIC OF CHINA once you finish your physical exam.These documents MUST be received prior to the students moving onto campus. So please email the Health packet to health@webster.edu and cc to Webster Staff.
  

• Book Air tickets.  (Move in  Webster Village Apartment around May 22 and Move out  on Aug. 1,2012  ) Please double check with Webster Staff about exact date.
  
• Call Center : Applicant need purchase the pre-paid PIN card that allows you to make an appointment through the Visa Information Call Center at most CITIC  branches and Candidates set appointments, individually, with the U.S. Consulates in Shanghai, Shenzhen and Chengdu around at the end of April or early of  May. Please inform the Webster office your interview appointment number and date.
• Webster office will issue a letter and fax our group interview appointments to U.S. Consulate before you take interview.

Step 2:

• Get invitation letter and DS2019 form (Exchange students) from Webster at the end of March.  
• Fill out the Webster Village Apartment Application Form ,Scan and Email back to Wester Staff.
• Download the Introduction how to pay SEVIS fee  and  Pay SEVIS Fee J Visa
• Please go to https://www.fmjfee.com/i901fee/index.html to pay SEVIS fee $180 by Credit Card online. 
• Pay visa fee at CITIC Bank aroud $140
• Prepare visa application documents.

 Step 3:

• Interview in U.S. Consulate. 
• Issue the air tickets and send flight information to Webster Staff 
• Confirm the Tuition difference and other costs with Webster Staff.
